• To be entered into the draw, the answers must be complete and accurate
• It is open to clinicians and individuals aged 18 or over
• The patient's permission to share their story must be obtained
• One completed submission per month will be selected as the winner who will be contacted via email or phone.
• The winner consents to their name and region being used on Seating Matters marketing activities via website, social media and other channels
• Seating Matters decision is final and no correspondence will be entered into.
